How to complete this form.

Please note - You cannot save the application form part way through and return to complete at a later date. You will need all of the information shown below to complete the form (the application should not take more than 10 minutes).

Please complete all relevant sections of the form

  • Qualifications - please include digital copies of your qualification certificates where possible. If the chosen category has a mandatory qualification (see below) we have to see a copy of the qualification.

  • Self-declaration - All questions in Part 8(a) (declaration) of the application form must be answered. If you answer ‘yes’ to any question you should provide a detailed explanation of your declaration in section 8(b). Failure to do so will result in your application form being rejected.

  • Payment via Card is required to complete the application.

What happens once we’ve received your application?

We’ll try to process applications within three to five working days but it may take longer at busier times of the year. Incomplete forms, or missing documentation, will result in the process taking longer.

After we have received your application we will undertake a number of security checks.


Who needs to register?

We have seven registration categories and you may have to register in more than one category, depending on the work you do or intend to do. You only have to pay one fee (i.e. the highest amount).

*If you are working towards one of the mandatory qualifications, you will be provisionally registered whilst you train.

When submitting an application for registration you are required to pay an application fee (see above). Please note, we are unable to consider any application for registration submitted without a payment. If no payment has been made (excluding an employer salary deduction in March), the application form will be rejected and a new copy will need to be resubmitted. The application fee is non-refundable in most cases. Once our assessment of you application for registration has been completed, and if granted, you will receive confirmation of your registration by email listing the categories that have been applied to your record. Please check your spam folder on a regular basis.

Tick the box if you are currently working unregistered but have received a salary deduction from your employer in March. Please note that this does not apply to applicants employed by Supply Agencies.

 

Annual renewal of registration before 31 March each year.

All registered practitioners will pay an annual registration fee. The fee is due by the 31 March each year and is set by the Welsh Government.
